After reading the Official Rules and Privacy Policy, download the ZIP file. This file will contain the Release and the Affidavit. Fill out the Release, the Affidavit, attach a photograph that should show entire face and full length of a clothed body, and some form of Personal Identification (driver license, valid passport, etc.) to verify minimum age of 21 years old. When you have the Release, the Affidavit, the Photo, and a copy of your ID; submit the information several different ways:
